Due early in the semester of graduation:

• Application for Degree: consult the Office of Graduate Studies website for exact deadline

Due Second Semester:

• Selection of Ph.D Supervisory Committee Form
• Program of Studies Instructions and Sample
• Program of Studies

Fourth Semester:

• Research Update Interview (RUI) Examination Report
• RUI/OPO/MS & Ph.D Defense Assessment: ask your supervisory or defense committee members to complete the online Student Assessment Form.

Fifth Semester:

• Original Proposal Oral Examination (OPO) Submission Form
• OPO Examination Report
• Admission to Candidacy (PhD)
• RUI/OPO/MS & Ph.D Defense Assessment: ask your supervisory or defense committee members to complete the online Student Assessment Form.

Year 4 and 5:

• Guidebook for preparing Thesis or Dissertation
• Full-time Certification Form

Semester of Graduation:

• Application for Final Oral Exam or Waiver (PhD) – at least 3 weeks before oral exam
• Application to Degree – very early in the semester: check the Office of Graduate Studies website
